Seeing a GP or Clinician at Crown Heights
At Crown Heights Medical Centre, patients are registered with the practice as a whole, not a specific doctor.
Although every patient has a ‘registered GP’ listed on their record, this is for administrative and NHS list purposes only. It does not guarantee appointments with that GP.
We work as a large multi-disciplinary team, which means you may be seen by a GP, paramedic practitioner, nurse practitioner, pharmacist, or another appropriate clinician depending on your needs. This helps us offer appointments sooner, ensure safe care, and direct you to the right specialist for your condition.
Requesting to Change GP
We understand that patients sometimes have preferences about who they see, but we do not provide a named or family GP service. Requests to change your ‘registered GP’ are only considered when:

There has been a significant breakdown in the clinical relationship, or

You have a clinical need for continuity due to complex or ongoing medical conditions.
Preference alone (e.g. wanting a different style, personality, or routine appointments with one doctor) is not a valid reason for a GP change.
If you have concerns about a consultation or a member of our team, please let us know so we can address the issue directly.
Can I Request Not to See Someone?
If you feel unable to continue care with a specific clinician, you can tell us. We may ask for some detail so we can investigate any concerns fully. In some cases, we may offer a different clinician to support your care, without changing your registered GP.
Continuity of Care
Where clinically appropriate (for example, complex health needs, end-of-life care or specific long-term conditions), we will do our best to provide continuity appointments with the most suitable clinician.
Our Priority
Our priority is to make sure you receive timely, safe and appropriate care from the most suitable member of our clinical team.
